1. Gather Required Documentation
- Form DS-11, completed but not signed
- Note: When completing your child's form by hand, please list on Address Line 2 "In Care Of" your name or the name of the child's other parent or legal guardian. By providing “In Care Of” information, you will ensure that your child's passport is not returned to us if the delivery service does not recognize your child's name as someone who normally gets mail at your address.
- Evidence of child's U.S. citizenship
- Photocopy of child's evidence of U.S. citizenship
- Evidence of parental relationship
- Parents'/guardians' photo ID
- Photocopies of those photo IDs
- Evidence of parental consent
- Passport photo
2. Total Fees
- Passport Card $15.00
- Expedited at Agency $60.00
- Execution Fee $35.00
- Payable to Department of State $110.00
